The Practice contracts with NHS Tayside Board to provide the following services, which are more fully described in the General Medical Services Contract. Patients should note that they will be asked to attend the appropriate healthcare professional when accessing these services, which may not always be the GP.
All GP practices are contracted to provide core services for their patients. The practice also provides the following:
If you are a patient living with a chronic disease (e.g. diabetes, asthma, COPD, hypertension, mental health issues, stroke, ischaemic heart disease, rheumatoid arthritis and epilepsy) we help by monitoring your health. Sometimes this involves reviewing things here at the practice.
For a lot of people with chronic diseases, what you do day-to-day can make a big difference to the management of your condition and the quality of life you experience. Making changes to your lifestyle may be a part of this, which may include changing what you're eating, stopping smoking, being more active and reducing your alcohol intake. It can be hard to make changes and know where to start. Asthma Management: By appointment with the Practice Nurse.
Baby Clinic: Mothers can attend any of the drop-in baby clinics held at the Forfar practices:
The Health Visitor can also see parents/babies on an individual basis at Ravenswood Surgery – please contact the Health Visitor to arrange.
Child Immunisations – routine appointment with the GP in the first instance and the immunisation team thereafter. Please note that parental consent is required before any immunisations can be given.
Cervical Smears: Appointments with the practice nurse are available for this.
Contraception/Family Planning: GPs, Practice Nurses and Health Visitors all give family planning advice including emergency contraception. Dr Catriona MacLean is also trained in Implanon and intra-uterine contraceptive devices.
Diabetic Management: By appointment with the Practice Nurse and in conjunction with Ninewells Hospital where needed.
Diet Advice: By appointment with the Practice Nurse. The Health Visitor can also provide information and advice on health related topics e.g. smoking, alcohol and exercise.
Immunisations/Vaccinations: Appointments will be sent out by the Immunisation Team; 1st child immunisation require an appointment with the GP beforehand.
New Patient Medical: By appointment with the Health Care Assistant within one month of registering. This will be the only medical information available until your notes are sent to us.
Travel Immunisations: Patients should request a travel form from reception and complete this 12 weeks prior to the appointment. Certain immunisations such as yellow fever will require to be carried out elsewhere (a charge is applicable). If your travel plans are complex, or you are travelling soon, you may be asked to attend a private Travel Clinic for advice and treatment. You will be charged for their services.
Treatment Room: By appointment with the Practice Nurse or Health Care Assistant for blood test, injections, dressings, blood pressure readings, etc. Where possible appointments before 11am are kept for blood samples which have to be sent to the lab.
Wart Clinic: By appointment with the Health Care Assistant every month.
Weight Management: By referral from your GP and by appointment with the Practice Nurse .
Paediatric Screening: All GPs are qualified to assess pre-school children. The Health Visitor will contact you and advise the parent to make an appointment with the GP to complete this.
Physiotherapy: Either self-referral or referral from the GP to services based at Whitehills Health and Community Care Centre.
Postnatal Care: Please advise the Practice on discharge. The Community Midwife, Health Visitor and GP provide care in the home. A 6-week postnatal appointment is strongly recommended and new mothers should make this appointment.
Psychologist: A psychologist from Angus Adult Psychological Therapies Team can be seen by referral.
Sleep Clinic: By referral from the GP.
Smoking Cessation: All local Pharmacies deliver smoking cessation advice and support.
Stress Management: By referral from the GP to either the Health Visitor, Counsellor or Psychologist.
Warfarin monitoring: Once stabilised patients will be referred to the service operated from Whitehills Hospital

ADVICE & VACCINATIONS
If you have plans to travel abroad for work or your holiday it is always good to be prepared well in advance. This is especially important if you might need vaccinations which can sometimes take up to 8 weeks to complete the course. We advise you to start planning to protect your health whilst on holiday 12 weeks in advance of your trip whenever possible.
Ravenswood Surgery are keen that you receive the best advice about your travel and are able to access all necessary vaccinations in advance of your trip. The NHS only provides certain vaccinations for patients planning to travel abroad. These vaccinations are listed below as well as a selection of other vaccinations and treatments that are not covered by the NHS and you will need to source privately.
Unfortunately, Ravenswood does not offer a private, non-NHS, travel vaccination service but is happy to provide you with details of where you can source high quality information about your trip, whether you need vaccinations and also a list of local providers of private travel services. We will also be happy to provide you with a list, as contained within your medical record that we hold, of your vaccination history.

Travel Advice & Planning
The best place to start in your preparations for a trip abroad is to get advice on how you should prepare, including which vaccines you may need. The following websites provide high quality, up to date and reliable information to help you plan. Expert Travel advice can easily be found online or over the phone.
These websites will give you an indication of whether you require any vaccinations, or medication to protect you from Malaria. In many cases you will already have had vaccinations and we will be happy to provide you with a list of the vaccinations that are recorded in your medical record. If you require vaccination, then you can access this as detailed below. Some vaccines are not provided on the NHS so you will need to arrange for this through a Private Travel Clinic.
Please note that vaccinations for business travel are not available on the NHS and should be organised by the employer through a private clinic.
We can give you a printout of immunisations from your electronic record, please ask at Reception.
Private Travel Clinics
If you want a ‘one stop shop’ for all advice and vaccination you may visit a Private Travel Clinic. If you require vaccinations that are not provided by the NHS then you will need to attend a private travel clinic as these are not available at the GP surgery. Many patients choose to gain initial advice as well as vaccination at these clinics.

You may be recommended to get vaccines provided on the NHS by these clinics. If that is the case, please request a prescription from us for the NHS vaccinations advised by the travel clinic. Any other, non NHS, medication/vaccinations required must be sourced through the Private Travel Clinic.
If you do require vaccinations that are provided by the NHS, you should then contact us at the surgery to make arrangements for vaccination. These vaccinations are detailed below.
Vaccination will be delivered by our practice nurse. Please ensure that you do this with plenty of notice to allow immunisations to be delivered and become effective before your trip. We recommend at least 8-12weeks ahead of your planned travel.
Please complete the Ravenswood NHS vaccination service request below and hand this to reception.
We will arrange a prescription for the required vaccines and ask the chemist to send them to us in the Practice. We will inform you when the vaccine is available and offer you an appointment for this.
Protects for 25 years – if you have 2 vaccines 6m -1 year apart
(if you are not sure you have had this a blood test can be done to check)
Protects for 3 years
Protects for 10 years (if you have received routine childhood vaccines)
Advised to repeat after 6m-2 years depending on age group
Can be considered if travelling to high risk area and not fully protected
Many vaccinations and medications that are required for travel are not available as part of the NHS service. These vaccines and medications will be provided on a private basis and you will need to pay for these. Ravenswood Surgery does not provide private travel services. The providers detailed above will be able to help you with these requests.

Some services provided are not covered under our contract with the NHS and therefore attract charges. Examples include the following:
The fees charged are based on the British Medical Association (BMA) suggested scales and our reception staff will be happy to advise you about them along with appointment availability.
Health Data Collection and Health Observatory
The staff at this Practice record information about you and your health so that you can receive the right care and treatment. We need to record this information, together with the details of the care you receive, so that it is available each time we see you.
The information recorded about you may also be used for reasons other than your personal care, for example, to help to protect the health of the general public, to plan for the future, to train staff and to carry out medical and other health research or studies.
Thus, we are involved in a health observatory based on anonymised patient information. The reasonable security measures and anonymisation processes are in place to comply with the relevant data protection regulations. If you would like opt out of this data collection scheme, please let your doctor know and your records will not be collected for use for this Observatory. This will not affect your care in any way.
If anything to do with the Observatory or derivative research would require that you provide additional information about yourself, you will be contacted by your GP to see if you are willing to take part; you will not be identified in any published results.
You have a right of access to your health records. If at any time you would like to know more, or have any concerns about how we use your information, please ask reception for more details.

Data Protection Notice
Ravenswood Surgery processes personal identifiable information that relates to patients and is therefore required by law to comply with the General Data Protection Regulations (GDPR), which protect your privacy and ensure that your personal information is processed fairly and lawfully.
We use personal information on different groups of individuals including:
The personal information we use includes information that identifies you like your name, address, date of birth and postcode.
We also use more sensitive types of personal information, including information about racial or ethnic origin; political opinions; religious or philosophical beliefs; genetic and biometric data, health; sex life or sexual orientation.
The information we use can relate to personal and family details; education, training and employment details; financial details; lifestyle and social circumstances; visual images; details held in the patient record.
We use personal information to enable us to provide healthcare services for patients; research; supporting and managing our employees; maintaining our accounts and records.
Ravenswood Surgery as data controller is required to have a legal basis when using personal information. Ravenswood Surgery considers that performance of our tasks and functions are in the public interest. So when using personal information our legal basis is usually that its use is necessary for the performance of a task carried out in the public interest, or in the exercise of official authority vested in us. In some situations we may rely on a different legal basis; for example, for compliance with a legal obligation to which Ravenswood Surgery is subject to, for example under the Public Health etc (Scotland) Act 2008 we are required to notify Health Protection Scotland when someone contracts a specific disease.
When we are using more sensitive types of personal information, including health information, our legal basis is usually that the use is necessary:
When you do not provide information directly to us, we receive it from other individuals and organisations involved in the delivery of health and care services in Scotland. These include NHS Boards and primary care contractors such as dentists, pharmacists and opticians; other public bodies e.g. Local Authorities and suppliers of goods and services.
Depending on the situation, where necessary we will share appropriate, relevant and proportionate personal information in compliance with the law, with the following:
It is sometimes necessary to transfer personal health information overseas for example if you require urgent medical treatment abroad. When this is needed information may be transferred to countries or territories around the world. Any transfers made will be in full compliance with NHSScotland Information Security Policy.

You have the right to access your own personal information.
This right includes making you aware of what information we hold along with the opportunity to satisfy you that we are using your information fairly and legally.

Although we must provide this information free of charge, if your request is considered unfounded or excessive, or if you request the same information more than once, we may charge a reasonable fee.

Once we have received your request and you have provided us with enough information for us to locate your personal information, we will respond to your request without delay, within one month. However If your request is complex we may take longer, by up to two months, to respond. If this is the case we will tell you and explain the reason for the delay.
The right to rectification
If the personal information we hold about you is inaccurate or incomplete you have the right to have this corrected.
If it is agreed that your personal information is inaccurate or incomplete we will aim to amend your records accordingly, normally within one month, or within two months where the request is complex. However, we will contact you as quickly as possible to explain this further if the need to extend our timescales applies to your request. Unless there is a risk to patient safety, we can restrict access to your records to ensure that the inaccurate or incomplete information is not used until amended.
If for any reason we have shared your information with anyone else, perhaps during a referral to another service for example, we will notify them of the changes required so that we can ensure their records are accurate.
If on consideration of your request Ravenswood Surgery does not consider the personal information to be inaccurate then we will add a comment to your record stating your concerns about the information. If this is case we will contact you within one month to explain our reasons for this.
If you are unhappy about how Ravenswood Surgery has responded to your request for rectification we will provide you with information on how you can complain to the Information Commissioner’s Office.
The right to object
When Ravenswood Surgery is processing your personal information for the purpose of the performance of a task carried out in the public interest or in the exercise of official authority you have the right to object to the processing and also seek that further processing of your personal information is restricted. Provided Ravenswood Surgery can demonstrate compelling legitimate grounds for processing your personal information, for instance; patient safety or for evidence to support legal claims, your right will not be upheld.
