New Patient Registration

We are currently only able to accept new registrations from those who are new to this area.

Patients must provide proof of identification and their address when completing registration forms. Registration forms are available at the reception desk or can be completed online on the above “Registering Online” page.

Please note that forms must be printed and brought to the Practice so that the receptionist can witness them being signed and to check your identification documents. You will also be asked to complete further Practice paperwork to complete your registration.

Registration Procedure

• Complete and sign your Medical Card and return to reception.
• A form GPR can be obtained if you have lost your Medical Card, this can be obtained from the receptionist.
• The receptionist will issue you with a New Patient Questionnaire to complete.
• This will be assessed by one of our GPs who will decide with whom you should arrange a New Patient Medical.
• You may be asked to make an appointment for a New Patient Medical with the appropriate healthcare professional.

Practice Area

When you are registering or moving house you might be asked to register with a Practice nearer to you.  Please check with the surgery to ensure you are within our practice boundary before completing your registration.  A more detailed map is available for inspection at the surgery.

Preference of Practitioner

Patients have the right to express a preference to receive services from a particular healthcare professional.  A verbal expression can be made at the time the appointment is being arranged.  It should be noted however that this might not always be possible if the practitioner requested cannot provide service.  Patients should note that there may be a delay in accessing services if they request a specific practitioner who is on annual or sick leave.

Change of Personal Details

You should notify the Practice immediately should you or any family member change address and/or telephone number. If you move outwith the catchment area of our practice you will be asked to register with a Practice, which is nearer to you.
